Job Description
Registered Nurse will:
- Provide direct patient care in home setting taking role as case manager
- Work in multi-disciplinary team to provide full range of home health services including assessing, planning, coordinating and evaluating plan of care under the orders of a physician
- Participate in team-based quality improvement, supervises C.N.A.
Access to protected health information is limited to the minimum necessary to perform the duties of the job.
Qualifications
- Current Nurses Illinois license
- Current BLS
- Valid Illinois driver’s license with dependable transportation
- Proof of auto insurance
- Ability to work independently, make sound decisions and proficient in computer skills.
- Proficient in IV’s insertion, lab draws and works effectively in a team atmosphere
- Excellent communication skills and portray a positive attitude.
- Able to lift up to 70 pounds occasionally and have the ability for frequent stooping, kneeling, bending, twisting and turning
- Position requires continuous walking and standing
- Minimum of 2 years’ experience in an acute care setting with home health experience preferred
Grade 20.
