COVID-19 Updates - 2022
12/21/2022 - As of this morning, we have successfully completed 14 days without any new positive cases for staff or residents. LTC will no longer be in outbreak status. Residents will no longer be routinely tested weekly but will continue to be tested if they are symptomatic or have had an exposure. Staff will continue to test and mask in accordance with CMS and IDPH requirements. We ask that visitors refrain from visiting if they have had an exposure to someone who is positive, have any symptoms of COVID, or who have had a positive test and are still within the healthcare criteria to end isolation. Masking is strongly recommended for all visitors who enter LTC during outbreak status or when community transmission is high. Please perform hand hygiene when entering the facility and frequently while in the facility to further minimize risk. HHH LTC will continue to do our very best to follow best practices to reduce spread and ensure that any positive residents receive the best care possible.

12/16/2022 - Weekly testing of residents and staff have been completed with no new cases found. We will continue to test residents at least weekly until we are out of outbreak status. 12/9/2022 - Yesterday we were notified that an ancillary employee tested positive for COVID-19. All residents will test again on Tuesday 12/13/22. We will continue to test residents at least weekly until we are out of outbreak status. We will also perform immediate testing of any symptomatic resident. Staff will test in accordance with CMS and IDPH requirements until we have assured that testing is no longer necessary. 12/5/2022 - Yesterday afternoon we were notified that an ancillary employee tested positive for COVID-19. All residents will test Tuesday 12/6/22. 12/3/2022 - This morning we were notified that an ancillary employee tested positive for COVID-19. All residents will continue to be tested weekly on Friday. Resident testing was completed on 12/2/2022 and no positives were noted. We will continue to test residents at least weekly until we are out of outbreak status. We will also perform immediate testing of any symptomatic resident. Staff will test in accordance with CMS and IDPH requirements until we have assured that testing is no longer necessary. We ask that visitors refrain from visiting if they have had an exposure to someone who is positive, have any symptoms of COVID, or who have had a positive test and are still within the healthcare criteria to end isolation. We will continue to test residents at least weekly until we are out of outbreak status. We will also perform immediate testing of any symptomatic resident. Staff will test in accordance with CMS and IDPH requirements until we have assured that testing is no longer necessary. 11/7/2022 - As of yesterday, we have successfully completed 14 days without any new positive cases for staff or residents. LTC will no longer be in outbreak status. Residents will not be tested weekly at this time and will only be testing based off symptoms or exposures. Staff will continue to test based off CDC and IDPH regulations. Effective immediately, LTC will adopt CDC guidance and discontinue universal masking requirements for employees due to the COVID-19 public health emergency. Staff will be required to remask if certain requirements are met such as high community transmission level, outbreak status, taking care of residents on isolation requiring masking, or other case-by-case scenarios. HHH LTC will continue to do our very best to follow best practices to reduce spread and ensure that any positive patients get the best care possible. 10/31/2022 - Weekly testing of residents occurred this morning with no new cases found. We will continue to monitor residents with weekly testing on Mondays during outbreak status and immediate testing of any symptomatic resident. Staff will continue to test in accordance with CMS and IDPH requirements until we have assured that testing is no longer necessary. 10/25/2022- On October 14, 2022, Illinois Executive Order 2022-21 was issued to eliminate the vaccination or testing requirement for Illinois health care workers and to eliminate the Illinois-specific face covering requirement for health care facilities to align with federal guidance. At this time, we are strongly recommending that any visitor entering Long Term Care during an outbreak wears a mask when in the facility. We also encourage that a mask is worn when visitation occurs outside of the resident’s room. Staff will continue to wear masks during outbreak status, if community transmission is high, in rooms of any residents with suspected or confirmed COVID, or if they have had close contact or a higher-risk exposure. Masking of staff will not be required if all of those criteria are not met. When Long Term Care is not in outbreak status, asymptomatic up to date and not up to date staff will no longer be routinely tested.
Please see information below regarding Updated Health Care COVID-19 Requirements:
Executive Order 2021-12 was amended and revised to align with Centers for Disease Control and Prevention (CDC) healthcare guidance CDC COVID-19 Infection Prevention and Control Recommendations released on September 23, 2022, and adopted by federal Centers for Medicare & Medicaid Services (CMS) for nursing homes in QSO-20-39-NH. When the SARSCoV-2 Community Transmission Level for the area is high, CDC now recommends source control for everyone in health care settings when they are in areas of the health care facility where they could encounter patients. When SARS-CoV-2 Community Transmission levels are not high, health care facilities may choose not to require universal source control. However, even if source control is not universally required, it remains recommended for individuals in health care settings who:
- Have suspected or confirmed SARS-CoV-2 infection or other respiratory infection (e.g., those with runny nose, cough, sneeze).
- Had close contact (patients and visitors) or a higher-risk exposure (HCP) with someone with SARS-CoV-2 infection for 10 days after their exposure.
- Reside or work on a unit or area of the facility experiencing a SARS-CoV-2 outbreak. Universal use of source control could be discontinued as a mitigation measure once no new cases have been identified for 14 days.
- Have otherwise had source control recommended by public health authorities.
Regardless of the Community Transmission level, CMS allows residents and their visitors when alone in the resident’s room or in a designated visitation area to choose not to wear face coverings or masks.
Executive Order 2021-22 (Vaccination and testing requirements) was modified to eliminate Section 2, which previously required health care workers in Illinois to be vaccinated or undergo regularly testing. This change aligned with CDC COVID-19 Infection Prevention and Control Recommendations released on September 23, 2022 and adopted by federal CMS for nursing homes in QSO-20-38-NH, which were both modified to no longer recommend routine testing of asymptomatic staff. Illinois-mandated vaccination and testing requirements have thus been lifted for health care facilities except for state-owned and state-operated facilities. Health care facilities should still continue to follow any applicable federal law and regulations, including federal regulations from CMS that still require staff at CMS-certified facilities to be vaccinated unless they have an exemption. Sections 5, 7, 8, and 9 of Executive Order 2021-22, as amended by Executive Order 2021-23 and Executive Order 2021-27 were re-issued and extended through November 12, 2022.
Employers or facilities may choose to implement additional strategies in order to protect staff and residents from COVID-19 and other respiratory viruses.
10/24/2022- On Sunday evening we were notified of a positive contract employee who tested positive, though they hadn't been in facility for greater than 48 hours from positive result. Resident testing was completed on all residents today and no positive cases were found. We will continue to monitor residents with weekly testing on Mondays and immediate testing of any symptomatic resident. Staff will continue to test in accordance with CMS and IDPH requirements until we have assured that testing is no longer necessary.

Staff will continue to test in accordance with CMS and IDPH requirements until we have assured that testing is no longer necessary. 7/18/2022- This morning, we were notified that an ancillary employee tested positive for COVID-19 on Friday 7/15/2022. At this time, we will continue to monitor residents with weekly testing on Thursdays. Testing may be increased at any time to twice weekly and will continue to test any symptomatic residents immediately. Staff will continue to test in accordance with CMS and IDPH requirements until we have assured that testing is no longer necessary. We continue to ask that if you are experiencing ANY symptoms or are aware of ANY exposure that you may have received, please wait to visit your loved ones until you have given enough time to mitigate risk. Visitors must follow the quarantine and isolation guidance for LTC residents listed below (shortened CDC time periods for general public do not apply):
- No visitor should visit for 10 full days after a positive test regardless of their vaccination status.
- Visitors who are up to date with COVID-19 vaccinations and had close contact with a positive case are allowed to visit as long as they remain asymptomatic
- Visitors who are not up to date with COVID-19 vaccinations and had contact with a positive case should not visit for 10 full days following the last exposure
With that said, all visitation that passes initial screening and can follow mask wearing, will continue to be welcomed to enter.
7/12/22: On 7/12/2022, Oregon Healthcare Pharmacists were in LTC to complete a COVID vaccination clinic for residents and staff. We appreciate the continued support from residents and families to ensure our residents remain up to date with COVID vaccines as we try to try to reduce the spread of COVID-19.
7/13/22: Yesterday evening, we were notified that an ancillary employee tested positive for Covid-19. 6/7/22: This morning we were made aware of 3 employees testing positive for Covid-19. 2/25/2022: During out weekly testing on 2/24/22 and 2/25/22, we found no residents or employees positive for COVID-19. This testing concludes our outbreak testing as we have been Covid negative for 14 days. We will no longer be testing vaccinated residents or staff weekly. They will return to symptomatic testing only at this time. HHHLTC will do our very best to follow best practices to reduce spread and see any positive patients get the best care possible. With that said, all visitation that passes initial screening and can follow mask wearing, will continue to be welcomed to enter per CMS’s new regulation from 11/12/21.
2/11/22: We found out yesterday that we have two positive staff members. One of the employees had not been to work for more than 48 hours. All residents were tested for routine weekly testing and no new positive cases were found for residents. Also, the last 2 COVID residents were moved from the COIVD unit and back to general population on 2/7/2022. Residents will continue to be tested every Thursday and staff will be tested every Friday. HHHLTC will do our very best to follow best practices to reduce spread and see any positive patients get the best care possible. We ask that if you are experiencing ANY symptoms or are aware of ANY exposure that you please wait to visit your loved ones until you have given enough time to mitigate risk. 2/4/2022: During the week we have found 4 LTC employees to be positive for COVID. 1 staff member on each of the following dates: 1/30/22, 2/2/22, 2/3/22, and 2/4/22. All residents were tested on 2/3/222 and no new positive cases were found. Two residents remain on the COVID unit and will be returning to general population on 2/7/22.
