20th Annual Foundation Golf Outing

Funds raised will go toward the purchase of a 3D Diagnostic Angiogram Workstation. 

Sponsors are being sought for this event. All sponsors will be listed on the sponsorship board and in the program. Please fill out the form below to register.

  • Masters - Food sponsorship - company name in program and signage...$2,000 (includes two foursomes on your team)
  • Eagle- Cart sponsorship - company name on each cart - $1,000 (includes one foursome and Mulligan Package)
  • Birdie- Event prizes, trophy and snacks sponsorship - $600 (includes one foursome)
  • Par- Multiple sponsorship – beverages, program printing, and other expenses - $300 (includes two golfers)
  • Hole- Hole sponsorship, signage at the tee, and the opportunity to set up a booth or table near the tee to distribute promotional materials - $100
  • Green- Signage at the green - $50
  • Hole Team - 4-person golfers and hole sponsorship recognition for the special price of $410 (4 x $90 plus $50 = $410)

Lunch will be served in the Clubhouse before shotgun start. After play, tacos will be served while we announce results and awards.

  • Longest drive (men's, women's)
  • Senior Longest drive (men's, women's 65 )

Team prizes will be awarded for men's, women's & mixed low gross.
