Hammond-Henry Hospital Rehab Department offers free physicals to Geneseo students
Free athletic school physicals for Geneseo students will be held on Thursday, May 2 from 5:30-7:30 pm at the Hammond Henry Hospital - Physical Therapy Department. Participants should enter at the East entrance of the hospital, turn left down the hallway, and take the elevator to the Second Floor. This offering is for all interested Geneseo incoming 7th, 8th, 10th, 11th, and 12th grade students. Attire should be shorts, t-shirts, but no socks.
Students need to bring a properly completed and signed physical form. You may pick up the IHSA physical form from the high school nurse’s office, high school main office, or online at www.ihsa.org/forms or www.geneseoschools.org. Complete the health history portion and have both the student and parent sign the form. Both the parent and the student signatures are required by IHSA. Students must have the proper signatures before being seen by the physician.
NO APPOINTMENT IS REQUIRED:
- Incoming 7th graders, please arrive at 5:30 pm
- Incoming 8th graders, please arrive at 5:45 pm
- Incoming Sophomores, please arrive at 6:00 pm
- Incoming Juniors, please arrive at 6:15 pm
- Incoming Seniors, please arrive at 6:30 pm
Please note that this physical will be good for 395 days and will cover all sports for the 24-25 school year. This would be a good opportunity to get a free physical for the entire year, even if you have a current physical that out dates during the next school year.
